Simulation and Live Fire Training - Waiver & Release of Liability

I, the undersigned, wish to participate in simulation training/live fire training provided by Artemis Defense Institute, and I am under no legal constraints that would prohibit me from participating in an activity or being in possession of a firearm.

Participation will include intense training exercises that could post a risk of injury to myself. I recognize and acknowledge that there is a risk of harm to my person, and I knowingly and voluntarily assume any and all such risks and hazards by agreeing to participate in the simulation training. I have fully disclosed any and all pre-existing medical conditions, such as epilepsy, heart conditions, spinal cord or nervous system issues, or any potentially dangerous medical condition.

I further acknowledge that I am at least 18 years of age, in good health, and knowingly assume the risk of injury of any and all kind arising out of my participation in the demonstration including, but not limited to, present and future damages for personal injuries or death, emotional distress, loss of income and any other types of monetary recourse against ADI.

I further agree and consent to release Artemis Defense Institute, their parent company and officers, directors and employees, and all persons or organizations, including partners connected with the organization, from liability for damages arising out of or suffered by the undersigned by reason of voluntary participation in the training exercise or use of ADI’s simulation equipment.

I also hereby grant to ADI, and its officials, agents, employees, assignees, and sub-contractors (collectively ‘ADI’) a perpetual license to prepare, use, reproduce, and publish images, photographs, film, videotape, and/or sound recordings of me, with or without the use of my name (the “materials”), for use in ADI sole discretion in conjunction with any simulation, editorial promotion, illustration, art, advertising, publicity, trade or any other lawful purpose, including but not limited to simulations, television, motion pictures, radio, magazines, publishing, advertising and marketing purposes, website usage, billboards, posters, product packaging, and point of purchase displays. I further waive any right of publicity I may have in the materials under any relevant law, regulation, or statute.
